Position Description
The case manager serves as counselor and resource person for a caseload of patients.
Specific duties include: providing individual and group counseling; participating as a member of a treatment team; corresponding with outside agencies, institutions and families of patients; collecting and writing social histories; monitoring patient/client treatment plan reviews; and compiling staffing notes.
Functions under the immediate supervision of a supervising psychologist, social worker, or physician.

Major/Essential Functions
  • Communicates with team members and supervisor in a timely manner utilizing direct complete and pertinent information in a professional and respectful manner.
  • Clinical documentation and correspondence contains professional language/terminology and is placed in the electronic health record as directed by policy.
  • Provides advanced case management services to patients who are receiving medical treatment or being observed in all areas of the Regional Medical Facility.
  • Contributes to the continuity of care for patients releasing to the community, including referral of hemodialysis patients to community dialysis clinics.
  • Participates as an essential member of the multidisciplinary team committed to the mission of maintaining access to care.
  • Maintains and updates the status of patients who are deemed seriously, critically ill and / or on hospice care and those receiving hemo-dialysis.
  • Provides supportive counseling to hospice patients and those who have experienced a significant change in their functioning or are facing that possibility. Follows clinical and security procedures when conducting any patient related activity.
  • Adheres to established guidelines for individual, psycho-educational and psychotherapeutic groups while following guidelines for individual and group demographics.
  • The Sr. staff serve as a role model to other staff members and therefore have added responsibility of demonstrating all the positive characteristics of a social worker and leader.
  • Supervise undergraduate/graduate social work students and provides supervision to staff in the absence of the supervisor.
  • Demonstrates a working knowledge of the information that needs to be entered in a patient’s medical record and  is familiar with the use of an electronic health record.
  • Maintains licensure as an LMSW/LCSW as required for this position.

Required Qualifications
Education/ Experience:
* Master's degree or equivalent from an approved accredited University.

* A minimum of six years related experience and/or training.


* Equivalent combination of education and experience to equal six years.

Credentialed as a Licensed Social Worker.
