Position Description
The case manager serves as counselor and resource person for a caseload of patients.
Specific duties include: providing individual and group counseling; participating as a member of a treatment team; corresponding with outside agencies, institutions and families of patients; collecting and writing social histories; monitoring patient/client treatment plan reviews; and compiling staffing notes. 
Functions under the immediate supervision of a supervising psychologist, social worker, or physician.


Major/Essential Functions
Communicates respectfully with team members and supervisor in a timely manner utilizing direct, complete, and adequate information. Communicates with social service and government agencies in a professional manner, upholding and adhering to privacy standards on behalf of the patients. Communicates effectively/productively with patients' family members as needed and permitted based on current policies. When problems arise they are addressed directly with the person(s) involved or speaks with their supervisor exclusively. Receives and is open to constructive feedback. Seeks feedback from others to improve clinical practice and safety.

Dependability - on time for treatment team, arranges coverage, notifies team members of absences. Scheduling - Patient's ITP's are reviewed based on timeframes stipulated in current policies and pertaining to release planning. Contribution- reports relevant information regarding O/P’s symptoms in context of the current DSM to corroborate diagnoses of record and current level of functioning so treatment teams can adjust treatment plans accordingly. Coordination-assists in the interviewing of O/P’s and other staff members using professional and assertive verbal/non-verbal communication. Preparedness-arrives with necessary materials to complete ITP's and special staffing notes.

Staff member monitors each offender patient prior to a treatment team review. Completes documentation. Meets standards for quality, quantity, time frames as set by supervisor and team. Conducts a patient interview on or before the 10th work day after the date of admission. The psychosocial evaluation and all corresponding documentation are completed per policy/supervisory directive. Adheres to clinical and security procedures at all times.

Responds to I-60's in accordance to existing policies. Responds to family calls per policy and using required forms. Completes release planning tasks according to policy and procedure. Manages treatment team referrals/referrals to specialized programs per policy.

Demonstrates knowledge of therapeutic approach and creates and maintains positive therapeutic environment. Maintains professional, therapeutic boundaries. Adheres to established guidelines for individual and group demographics. Adheres to established guidelines for individual, psycho- educational and psychotherapeutic groups.

Serves as and demonstrates knowledge of the patient advocate's role in representing the patient's right to refuse treatment in accordance with procedural hearings. Represents at the hearings the declared wishes of the patients, who may be living with severe mental illness and/or serious emotional disturbances, to the reviewing physicians who determine patient's need for inpatient psychiatric care. The clinician understands and demonstrates their role for a civil commitment hearing in a competent and professional manner. Documentation for each hearing is completed according to the associated policies.

Documents using legible handwriting and/or appropriate forms in the EMR, professional language, supports clinical assessment with objective information/observation, and avoids jargon. Follows policy and procedures in documenting group therapy sessions, row rounds, all clinical notes (i.e. SOAP notes),treatment plans, I-60’s, referrals and Civil commitment hearings.

Serves as and demonstrates knowledge of the non treating mental health professional's role and responsibilities in conjunction with the hearing for non emergency compelled medication. Attends / participates/ completes training required to meet requirements set forth by TTUHSC, TDCJ and TSBSWE.

Demonstrates knowledge and ability to document, find information and communicate effectively using the Electronic Medical Record, Microsoft Office Programs, TDCJ Host on Demand and programs/operations utilized by CMHC and TTUHSC.



Occasional Duties
Participates in committees or teams in order to enhance or create programs at the John T. Montford Unit. Participation in unit programs for staff, security, safety and support. Supervision of BSW students. Presentation or leadership in statewide or national professional organizations or conferences. Presentations for classes at local universities. Serves as certified Spanish speaking interpreter if TDCJ certification is secured.

Required Qualifications
+Education/ Experience:+ * Master's degree or equivalent from an approved accredited University.
OR
* A minimum of six years related experience and/or training.
OR
* Equivalent combination of education and experience to equal six years. +License/Certification:+ Credentialed as a Licensed Social Worker.


Preferred Qualifications
Is Licensed as an LMSW or LCSW in the state of Texas. Has experience working with people who are mentally ill. Has experience working in a correctional setting. Is able to effectively communicate in writing and verbally. Is capable of working on several tasks simultaneously and does so according to designated time frames and level of importance.

Employee is able to interact with people of all cultures, social economic backgrounds and educational levels. Understands and recognizes psychological, emotional and physical boundaries and practices in accordance with that knowledge and understanding. Is able to work in accordance with the standards of TTUHSC, CMHC, TDCJ and TSBSWE. Verbal and written communication is effective in the completion of clinical and administrative job tasks. Demonstrates an interest and ability to build upon existing skills and knowledge to enhance their professional growth and development, i.e. by accepting and using constructive criticism, seeking out training opportunities, etc.
